With the ladle she has been using to stir the contents of the bowl, she hits a man on the nose. The man wears a hat and carries an empty bowl. Caricatures of other people, including an old man in pinafores seated in a high chair and holding his nose as his prepares to eat a spoonful of the medicine, complete the illustration.
